A Well Use permit is required for any well located on a premise that is connected to the municipal water system in the City of Oak Creek.

Application Requirements

  • Plumbing Permit Application
  • A copy of the well inspection report (WI DNR Well Compliance Form 3300-305)
  • A copy of the approved well water test report
