The Special Needs Alert Program is a voluntary program that is designed to aid Oak Creek Police personnel in identifying and interacting with individuals with special needs that live, work or go to school in Oak Creek.  The Oak Creek Police Department developed the Special Needs Alert Form, that includes an individual’s picture, physical description, emergency contact information, and key information specific to the individual. 

We also hold a Special Needs Alert Program Event every 5 years, which allows parent/guardians the opportunity to listen to the presentation and enroll their loved ones with special needs int he program. We also offer department tours and donated snacks and refreshments from local businesses to our participants.   

The Special Needs Alert Form is completed by the individual or the individual’s guardian/caregiver and can be:

Mailed/Dropped off at the Oak Creek PD: 301 W Ryan Rd, Oak Creek, WI 53154 or;
Turned in to a School Resource Officer

Special Needs Event
