Should you choose, you may complete the form below and submit to the City Clerk’s office by mail, email or in person. All requests will be processed as soon as practicable and without delay.

To request a Statement of Real Property Status please click the link below:
You will then be prompted to fill out the online submission page for the property you are requesting. (You may request information for up to 5 properties within 1 submission.)
- After submission, you will then receive two emails. The first will be an automatic confirmation email stating your request has been successfully submitted, the second will have the invoice for the request(s) attached.
At this time, continue remitting payment (via check) to the City of Oak Creek.
Once the payment has been received AND the request has been completed by all departments, your requested information will be emailed to you.
